• Board Eligible (to become Board Certified within 2 years of becoming Board Eligible) or Board Certified in Internal Medicine - [REQUIRED]
• MI license (or willing to obtain) - [REQUIRED]
• DEA (or willing to obtain) - [REQUIRED]
• BLS, ACLS (or willing to obtain) - [REQUIRED]
• Clean malpractice - [PREFERRED ] Does the provider require visa support - J1 or H1B? (note in "Additional Submission Details" field) - [REQUIRED]
• If candidate is TRULY Board Eligible, must become Board Certified within 2 years of becoming eligible
